Grades & Types of Gynecomastia

Grade I — Minor Enlargement
Small amount of breast tissue localised to the peri-areolar area with no skin redundancy. The chest appears normal when clothed. Palpable firm disc behind the nipple is the defining feature.
Grade II — Moderate Enlargement
Moderate breast tissue that extends beyond the areola boundary. Visible chest contour change, noticeable through clothing. Causes significant embarrassment and lifestyle restriction. Most common grade treated.
Grade III — Significant Enlargement
Marked breast tissue with mild to moderate skin redundancy. The chest has a distinctly feminine contour. Often associated with prolonged duration and psychological impact. Requires combined surgical approach.
Grade IV — Severe Enlargement
The most advanced grade — marked breast tissue with significant skin excess and a fully feminising chest appearance. Often associated with obesity and requires the most comprehensive surgical planning.
